Medical records for past patients will be retained in accordance with the State of Texas medical information preservation guidelines. Former patients or their authorized representative can request records created at LifeCare through the following method:
Email authorization request and photo identification to pt.him-fax@Lifecare-health.com
All requests for medical records must be in writing and copy fees will be applied per Texas law.
