As of March 15, 2024, LifeCare Hospital of Dallas and LifeCare Hospital of Fort Worth are both closed.
To request Medical Records from one of these facilities, please review the following.

Medical records for past patients will be retained in accordance with the State of Texas medical information preservation guidelines.  After the date of closure, the responsibility for maintenance of the medical records will be transferred to LifeCare 2.0 Plano , which will act as the custodian of those records. All patient requests will be completed by our release of information vendor, MRO. Former patients or their authorized representative can request records created at LifeCare Ft. Worth through the following methods:

  1. Contact MRO directly via the contact information on the website.

  2. Call 214-473-3220 to request the Authorization paperwork, or

  3. You can mail a written request and photo identification to 6800 Preston Rd, Plano, TX 75024, or

  4. You can fax a written request and photo identification to 214-473-3222.

  5. Email authorization request and photo identification to fwt.medrec@lifecare-health.com.

All requests for medical records must be in writing and copy fees will be applied per Texas law.
